There were 58,443 births in 2021, 2,484 more births compared with 2020. This corresponds to a birth rate of 11.7 per 1,000 population, a rate increase of 0.5 from 2020.

There were 33,055 deaths registered in 2021, of these 17,212 were male while 15,843 were female. The 2021 total is 4.1% higher than in 2021 when 31,765 deaths were registered. See tables 1a and 10.

There were 180 infant deaths registered in 2021 giving an infant mortality rate of 3.1 per 1,000 live births.

The number of births less the number of deaths in 2021 resulted in a natural increase of population of 25,388 persons, 4.9% higher than the natural increase of 24,194 in 2020. See tables 1a and 1b.

Births

There were 58,443 births registered in 2021, 29,823 male births and 28,620 female births, an increase of 2,484 on 2020. The 2021 total is 21.7% lower than in 2011 when 74,650 births were registered.   

The 58,443 births represent an annual birth rate of 11.7 per thousand population. This rate is 0.5 per thousand population higher than in 2020. See tables 1a, 1b and 3. 

Fertility

The total period fertility rate, TPFR, is derived from the age specific fertility rates in the current year. It represents the projected number of children a woman would have if she experienced current age specific fertility rates while progressing from age 15-49 years. A value of 2.1 is generally considered to be the level at which the population would replace itself in the long run, ignoring migration.

In 2021 the TPFR for Ireland was 1.7 which is below replacement level. See table 2. 

Age of Mothers

The average age of first time mothers in 2021 was 31.6 years, up 0.2 years from 2020. The average age of mothers at maternity for all births registered in 2021 was 33.3 years, 0.2 higher than 2020. The average age of first time mothers outside marriage/civil partnership was 30.0 in 2021 while the average age of all mothers outside marriage/civil partnership was 31.1 years. The previous year the average age of mothers having their first baby outside marriage/civil partnership was 29.4 years while it was 30.7 years for all mothers for births registered outside of marriage/civil partnership. See tables 1a, 1b, 7 and 8. 

Births and marriage/civil partnerships

In 2021, there were 34,271 (58.6%) births registered as within marriage, there were no births within civil partnerships.

There were 24,172 births registered as outside marriage/civil partnership accounting for 41.4% of all births, an increase of 3.0% from 2020. The highest percentage of births outside marriage/civil partnership was in Limerick City at 55.6% and the lowest was in Dún Laoghaire-Rathdown with 27.6%. See table 6.

A total of 699 teenagers had babies in 2021, of these 16 were aged under 16. Over 42% of births outside marriage/civil partnership were to mothers under 30 (42.1%). See tables 5 and 8.

Location of births

The highest number of births registered was in Dublin City with 6,463 (11.1% of total live births in the country) followed by Cork County with 5,418 (9.3%) registered births. Leitrim had the lowest number with 405 (0.7%) registered births in 2021. The same pattern was reflected in 2020 when Dublin City accounted for 6,229 or 11.1% of births followed by Cork County with 5,100 (9.1%). Leitrim also had the lowest number, 380 births (or 0.7%), registered in 2020. See tables 3, 6 and 7.

Nationality of mothers

Of the 58,443 births in 2021, there were 45,381 babies (77.7%) born to mothers of Irish nationality compared to 43,019 (76.9%) in 2020. There were 8.4% of births to mothers of EU15 to EU27 nationality, 2.0% of mothers were of UK nationality, and 2.1% were of EU14 nationality (excluding Ireland). Mothers of nationalities other than Ireland, UK and the EU accounted for 9.8% of total births registered. There were 0.04% of mothers where the nationality was not stated. See table 9.

Birth Order

A total of 22,700 births (38.8%) were to first time mothers in 2021. Second time mothers had 20,341 births (34.8%). This accounted for a total of 73.6% of births of stated parity. See table 4 and figure 2.

Deaths

There were 33,055 deaths registered in 2021, of these 17,212 were male while 15,843 were female. The 2021 total is 4.1% higher than in 2020 when 31,765 deaths were registered. See tables 1a and 10.

Infant and neonatal mortality

There were 180 Infant deaths registered in 2021 giving an infant mortality rate of 3.1 deaths per 1,000 live births, an increase of 0.4 from 2020. Neonatal deaths are deaths of infants at ages under 4 weeks. There were 140 neonatal deaths registered in 2021, a neonatal mortality rate of 2.4 deaths per 1,000 live births, which was as increase of 0.4 from 2020. See tables 1a, 1b and 15, and figure 3.

Cause of death

Diseases of the Heart and Arteries:

Diseases of the circulatory system accounted for 8,753 deaths or an annual rate of 1.7 per 1,000 population. Of these, 4,121 were due to ischaemic heart disease and 1,423 to cerebrovascular disease. See tables 11 and 12, and figure 4.

Malignant Cancers:

There were 9,436 deaths from malignant neoplasms. This is equivalent to an annual rate of 1.9 per 1,000 population. See tables 11 and 12, and figure 4.

Lung Diseases:

Deaths from chronic lower respiratory disease numbered 1,443, pneumonia accounted for 677 deaths and 891 deaths resulted from all other respiratory diseases. See tables 11 and 12.

COVID-19 Virus Identified and Virus Unidentified:

There were 3,011 registered deaths where COVID-19 was assigned as the underlying cause of death in 2021. There were a further 495 mortality records where there was a mention of COVID-19 in the narrative of the death certificate and where the underlying cause of death was not COVID-19. Of these 495, deaths from neoplasms numbered 151 (30.5%) while diseases of the circulatory system accounted for a further 167 (33.7%) of these deaths. See tables 12, 13, 14 and 17. An information note is available outlining how the CSO assigns COVID-19 as the underlying cause of death.

COVID-19 Vaccine

In 2021, a death due to COVID-19 vaccines causing adverse effects in therapeutic use was recorded for the first time. This death occurred in the 75-84 age group: See tables 12, 13 and 14.

Accidents, suicides and other:

There were 1,428 deaths due to accidents, suicides and other external causes. Accidents accounted for 71.1% (1,016) of these deaths, while suicides accounted for 27.9% (399). Of these 1,428 deaths, 936 (65.5%) were male. See tables 12, 13, 14 and 16.

Late registered deaths:

Death due to intentional self-harm is classified as an unnatural death and therefore, must be referred to the Coroner for investigation. This investigation can take a protracted length of time to complete for various reasons (such as getting medical reports, health and safety reports, engineer’s report, the involvement of the Director of Public Prosecutions etc.) and this delays the registration of such deaths. There were 989 deaths registered late in 2021, of which 595 were male and 394 were female. There were 612 of these late registered deaths that occurred in 2019 and 377 that occurred in various years prior to 2019. See table B.

To address this problem the CSO publishes tables of late registered deaths by cause of death, sex and specific age-groups in the relevant Vital Statistics Annual Report. Numbers of deaths registered late due to intentional self-harm in 2021 will be included in the Appendix 2021 of the Annual Report 2021 and will be referred to the year that the death occurred i.e. 2019 and prior. These will be published in table 3.C and 3.D of that report which is scheduled for dissemination in October 2023. Therefore, any additional deaths due to intentional self-harm will be added to the deaths that were registered in time to be included in the relevant Annual Report.

For example:

There were 390 deaths due to intentional self-harm in 2019, 300 (or 76.9%) males and 90 (or 23.1%) females and were reported in table 3.17 of the Vital Statistics Annual Report 2019. Any death assigned an underlying cause of death that is included in the 612 late registered deaths in 2021, and where death occurred in 2019, will be added to the 390 figure of deaths due to intentional self-harm when the Vital Statistics Annual Report 2021 is published. Please refer to the CSO open data portal (tables VSD33 and VSD34) for the most up to date figures on deaths from intentional self-harm.

Age at death

The leading single cause of death in 2021 varied widely by age group. Among those aged from 15 to 34 years, external causes of death including accidents, suicide and other ranked first (251 deaths). Among individuals aged 35-74, cancers were the leading single cause group while for those aged 75 and older, it was diseases of the circulatory system. See table 12 and figure 5. 

There were 27,322 deaths of persons aged 65 and over registered in 2021. This represents an annual death rate of 36.8 per 1,000 population aged 65 and over and represents a rate increase of 0.1 when compared with 2020. See tables 1a, 1b and figure 5.

The average (mean) time-lag between the date of death and the registration of death was 67 days while the median (mid-point in data) was 36 days in 2021.

Marriages/Civil Partnerships

The number of marriages registered in 2021 was 17,217 of which 500 were same-sex marriages. This is equivalent to an annual marriage rate of 3.4 per 1,000 population, 1.5 more than the rate in 2020. See tables 1a and 1b.

Same-Sex marriage legislation came into effect on the 16th November 2015.

Civil Partnership legislation was introduced in Ireland on the 1st January, 2011.  From 16th November 2015, when same-sex marriages legislation came into effect, same-sex couples no longer had the option to apply for a Civil Partnership.

Population

The estimated population (usual residence) in April 2021 was 5,011,460. See table A.

Background Notes

Introduction

This report has been prepared by the Central Statistics Office for the Minister for Social Protection in accordance with the provisions of Section 2 of the Vital Statistics and Births, Deaths and Marriages Registration Act, 1952 and Government Order SI 831 of 2007.

Methodology:

All figures in this report are classified by year of registration. The Yearly Summary publication is a summary of the 4 quarterly publications for the reference year.

Deaths are classified to the World Health Organisation (W.H.O.) International Classification of Diseases, Version 10 (ICD10). 

The Underlying Cause of Death (UCOD) has been defined by the World Health Organisation (WHO) as

(a) the disease or injury which initiated the train of morbid events leading directly to death

or

(b) the circumstances of the accident or violence which produced the fatal injury.

From 1st January 2018 the CSO is using IRIS software to assign the UCOD.  IRIS is a European developed automated coding software, endorsed by Eurostat and is maintained by the IRIS institute and all W.H.O. updates to the ICD-10 classification are included according to the WHO timelines.

Inquest deaths are coded manually due to the nature of the reporting of these cases.  The difficulty lies in automatically assigning a verdict from a broad range of verdicts open to a Coroner or jury which include accidental death, misadventure, suicide, open verdict, natural causes (if so found at inquest) and in certain circumstances, unlawful killing.

Stated Parity 

Refers to the number of previous live births excluding any not stated.

Births Classified by Area of Residence of Mother

From January 2018 onwards where births take place within the State and the area of residence of the mother is outside the State, the area of residence of the mother is recorded as outside the State. Prior to this, births where the mother was residing outside the State were assigned to the location where the birth took place within the State.

Issue of Authority for Registration

Births and deaths (where no inquest has been held) which have not been registered within one year of their occurrence can be registered only on the authority of the Superintendent Registrars. The tables in the main body of this report exclude such births and deaths. The Appendix to the report contains an analysis of late death registrations in 2020 and of some deaths where an inquest has been held. The number of such deaths registered in 2020 was 444.

NUTS Regional Authority Areas 

From the 1st January 2018 an Amendment of the Eurostat Nomenclature of territorial units for statistics (NUTS classification) legislation came into effect. This reflects the new regional assembly structure and the other changes to the Local Government Act for Tipperary North, Tipperary South and Limerick and Waterford City and County Councils. The composition of the new Regions breakdown is outlined below.

Up to December 2017 regional classifications were based on the NUTS classification used by Eurostat. The NUTS3 regions corresponded to eight Regional Authorities established under the Local Government Act, 1991 (Regional Authorities) (Establishment) Order, 1993, which came into operation on 1 January 1994. The NUTS2 regions, which were proposed by Government and agreed by Eurostat in 1999, are groupings of the NUTS3 regions. The composition of the regions is set out below.

The European Union (EU) Countries

 The United Kingdom withdrew from the EU on the 31 January 2020. From the 1 February 2020 the EU is reduced to 27 Member States.

 As the United Kingdom was part of the EU for one month in 2020 - it is included as part of the EU 28 for quarter 1 2020.

Civil Partnerships

Civil Partnership legislation was introduced in Ireland on the 1st January 2011. From 16th November 2015, when same-sex marriages legislation came into effect, same-sex couples no longer had the option to apply for a Civil Partnership.  However, couples that had given the required 3 months' notice of a Civil Partnership prior to 16th November 2015 were still entitled to have a Civil Partnership ceremony after that date.  

Same-Sex Marriage Legislation

The Marriage Act 2015 (No. 35/2015) effective from the 16th November 2015 (S.I. 504/2015) refers, allows parties of the same sex to marry.

Usual Residence Population Concept

Rates for Q4 2020, were calculated using the relevant population data in April 2020.

For the purpose of this release the population concept of usual residence has been used, i.e. all persons usually resident and present in the State on census night, plus absent persons who are usually resident in Ireland, but are temporarily away from home and outside the State on Census night.  All persons are classified according to the region of their usual residence.

Revision of Rates

Rates for Births, Deaths and Marriages were revised in quarter 4 2017 for intercensal years 2012 to 2015, based on revised estimated usual residence population data. Rates for 2016 were also revised based on updated Census of Population usual residence population figures.  

Technical Notes:

Live Births

The terms used in relation to live births are defined as follows:

A live birth: is defined as the complete expulsion or extraction from its mother of a product of conception, irrespective of the duration of pregnancy, which, after such separation, breathes or shows any other evidence of life, such as beating of the heart, pulsation of the umbilical cord, or definite movement of voluntary muscles, whether or not the umbilical cord has been cut or the placenta is attached.

Parity: Parity is the number of previous live born children to a woman.

Birth Order: Birth order is the number  of live born children a woman has in the order in which they are born.

Parity and Birth order: A first birth relates to parity 0 or birth order 1 i.e. birth order = parity + 1.

Crude birth rate (CBR): The number of live births divided by the total population at that age (or age-group), multiplied by 1,000.

Age specific fertility rate (ASFR): The number of live births at a certain age (or age-group) divided by the female population at that age (or age-group) multiplied by 1,000.

Total period fertility rate (TPFR): The sum of the ASFRs, divided by 1,000 i.e. the expected number of children a woman will have in her lifetime based on the fertility of that year.

Formula for technical notes

Age specific reproduction rate (ASRR): The number of female live births at a certain age (or age-group) divided by the female population at that age (or age-group), multiplied by 1,000.

Gross reproduction rate (GRR): The sum of the ASRRs divided by 1,000 i.e. the expected number of daughters a woman will have in her lifetime based on the fertility of that year.

Net reproduction rate (NRR): The sum of the products of the ASRR's and the expected number of female years to be lived at that age, divided by 1,000 i.e. the GRR adjusted for female mortality.

The difference between the gross and the net reproduction rates is an indicator of female mortality.

Average age at maternity: The sum of the products of the ages at maternity of live births and their ages divided by the number of live births.

 Formula 2

Average age at maternity of first birth: The sum of the products of the ages at maternity of first live births and their ages divided by the number of first live births.

Childbearing years are regarded as between 15 and 49.

Live birth to mothers less than 15 are included in the age 15 category and are divided by the age 15 population. Similarly, live births to mothers aged greater than 49 are included in the age 49 category and are divided by the age 49 population.

Average of mean ages: Live births where the age at maternity is not stated are excluded from the calculation.

Deaths

Crude death rate (CDR): The number of deaths divided by the total population at that age (or age-group), multiplied by 1,000.

Standardised death rate : the (age) standardised death rate is a weighted average of age-specific mortality rates. The weighting factor is the age distribution of a standard reference population. The standard reference population used is the European standard population as defined by Eurostat in 2012. The new European Standard Population (ESP) is the unweighted average of the individual populations of EU-28 plus EFTA countries in each 5-years age band (with the exception of under 5 and the highest age group of 95+). The ESP is calculated on the basis of the 2010-based population projections, averaged over the period 2011-30.  

Stillbirths, Infant Mortality and Maternal Mortality

Stillbirth: Stillbirth weighing 500 grams or more or at gestational age of 24 weeks or more, this definition applies to stillbirth figures from 1995 onwards.

Late foetal death: Foetal death at or over 28 weeks gestation, this definition was applied up to 1994 inclusive.

Early neonatal death: Death at ages under 1 week live born infant.

Perinatal deaths: Stillbirths or late foetal deaths plus early neonatal deaths.

Late neonatal deaths: Death between the ages of 1 week and 4 weeks of live born infant.

Neonatal death: Death at ages under 4 weeks of live born infant.

Post neonatal death: Death between the ages of 4 weeks and 1 year of live born infant.

Infant death: Death at ages under 1 year of live born infant.

Maternal death: A maternal death is the death of a woman while pregnant or within 42 days of termination of pregnancy, irrespective of the duration and the site of the pregnancy, from any cause related to or aggravated by the pregnancy or its management, but not from accidental or incidental causes.

Stillbirth rate: Stillbirths divided by total live births plus stillbirths, multiplied by 1,000.

Late foetal mortality rate: Late foetal deaths divided by total live births plus late foetal deaths, multiplied by 1,000.

Early neonatal mortality rate: Early neonatal deaths divided by total live births, multiplied by 1,000.

Perinatal mortality rate: Perinatal deaths divided by total live births plus stillbirths, multiplied by 1,000.

Late neonatal mortality rate: Late neonatal deaths divided by total live births, multiplied by 1,000.

Neonatal mortality rate: Neonatal deaths divided by total live births, multiplied by 1,000.

Post neonatal mortality rate: Post neonatal deaths divided by total live births, multiplied by 1,000.

Infant mortality rate: Infant deaths divided by total live births, multiplied by 1,000.

Maternal death rate: Maternal deaths divided by total live and stillbirths multiplied by 100,000.

COVID-19 as a Single Cause of Death

The number of COVID-19 deaths with no pre-existing conditions is different to COVID-19 as a single cause of death. Where COVID-19 is the single cause of death, no other condition is reported on the International Form of Medical Certificate of Cause of Death (see Example 1). However, it is possible for a person to have several conditions listed as causes of death. These may be pre-existing conditions or complications of COVID-19 (see Example 2). In Example 2, COVID-19 is the underlying cause of death¹ as defined by the W.H.O.

The CSO relies on certifiers to report conditions in the correct causal sequence which affects the selection of the underlying cause of death.
