The number of births increased by 2,666 (19.6%) while deaths increased by 1,228 (16.6%) compared to the same period in 2020.

There were 16,250 births in quarter 4 2021, 2,666 births or 19.6% more than the same period in 2020. This corresponds to a birth rate of 13.0 per 1,000 population, an increase of 2.1 as recorded in quarter 4 2020. 

There were 8,626 deaths in quarter 4 2021, an increase of 16.6% (or 1,228 deaths) from quarter 4 2020. This equates to a death rate of 6.9 per 1,000 population, an increase of 1.0 from quarter 4 2020.

There were 62 infant deaths registered in quarter 4 2021, giving an infant mortality rate of 3.8 per 1,000 live births.

The number of births less the number of deaths in quarter 4 2021 resulted in a natural increase of population of 7,624 persons.

Births

There were 8,335 male births and 7,915 female births in quarter 4 2021. The 16,250 births represent an annual birth rate of 13.0 per thousand population. An increase of 2.1 in the rate that was recorded in quarter 4 2020. See tables 4 and 5.

Age of mothers

The average age of first time mothers was 31.8, up 0.4 years from that recorded for the same period in the previous year. The average age of all mothers at maternity for births registered in quarter 4 2021 was 33.4 years, which is up 0.1 years from that recorded in quarter 4 2020. The average age of first time mothers outside marriage/civil partnership was 30.2, while the average age of all mothers outside marriage/civil partnership was 31.3 years. In the same quarter in 2020, the average age of mothers having their first baby outside marriage/civil partnership was 29.4 years while it was 30.9 years for all mothers for births registered outside of marriage/civil partnership. See tables 9 and 10.

Births and marriage/civil partnerships

In quarter 4 2021, there were 9,512 (58.5%) births registered as within marriage/civil partnership.

There were 6,738 births registered as outside marriage/civil partnership, accounting for 41.5% of all births in quarter 4 2021. The highest percentage of births outside marriage/civil partnership was in Limerick City at 55.9% and the lowest was in Dún Laoghaire-Rathdown with 28.9%. In the same quarter in 2020, there were 5,178 births registered as outside marriage/civil partnership accounting for 38.1% of all births. See table 8.

Location of births

The highest number of births registered was in Dublin City with 2,051 (12.6% of total live births in the country) followed by Cork County with 1,575 (9.7%) registered births. Leitrim had the lowest number with 101 (0.6%) registered births in quarter 4 2021. The highest number of births registered in quarter 4 2020 was Dublin City which accounted for 1,512 or 11.1% of births followed by Cork County with 1,283 (9.4%). Leitrim had the lowest number, 80 births (0.6%), registered in quarter 4 2020. See table 8.

Area of residence of mother

From January 2018 onwards, where births take place within the State and the mother is residing outside the State, the area of residence of the mother is recorded as outside the State. Prior to quarter 1, 2018, cases where the mother was residing outside the State, the mother's area of residence was assigned to the location where the birth took place within the State. 

There were 10 babies born to mothers who were resident outside the Republic of Ireland in quarter 4 2021. This accounts for 0.06% of total live births registered in the quarter. See tables 5, 8, 9 and 10. 

Nationality of mothers

Of the 16,250 births in quarter 4 2021, there were 12,540 babies (77.2%) born to mothers of Irish nationality compared to 10,427 (76.8%) in quarter 4 2020. See table 11.

Birth order

A total of 6,344 births (39.0%) were to first time mothers in quarter 4 2021 while 5,685 (35.0%) were to second time mothers. These accounted for a total of 74.0% of births of stated parity in quarter 4 2021. See table 6 and figure 2.

Deaths

There were 8,626 deaths registered in quarter 4 2021, of these 4,464 were male and 4,162 were female. The quarter 4 2021 total is 16.6% more than the corresponding quarter of 2020 when 7,398 deaths were registered. See table 12.

Infant and neonatal mortality

There were 62 infant deaths registered in quarter 4 2021 giving an infant mortality rate of 3.8 deaths per 1,000 live births, an increase of 0.9 points on that recorded in quarter 4 of 2020. Neonatal deaths are deaths of infants at ages under 4 weeks. There were 46 neonatal deaths registered in quarter 4 2021, giving a neonatal mortality rate of 2.8 deaths per 1,000 live births, which was an increase of 0.6 from quarter 4 2020. See tables 3, 4 and figure 3.

Cause of death

Diseases of the circulatory system:

Diseases of the circulatory system accounted for 2,281 deaths, or an annual rate of 1.8 per 1,000 population. Of these, 1,035 were due to ischaemic heart disease and 360 to cerebrovascular disease. See tables 2a, 2b, 13, 14 and figure 4.

Malignant neoplasms (Cancer):

There were 2,487 deaths from malignant neoplasms. This is equivalent to an annual rate of 2.0 per 1,000 population. See tables 2a, 2b, 13, 14, 15 and 16.

Diseases of the respiratory system:

Deaths from chronic lower respiratory disease numbered 403, pneumonia accounted for 187 deaths and 241 deaths resulted from all other respiratory diseases. See tables 2a, 2b, 13, 14, 15 and 16.

Accidents, suicides and other:

There were 523 deaths due to accidents, suicides and other external causes. Accidents accounted for 73.0% (382) of these deaths while suicides accounted for 25.8% (135). This figure of 135 reflects a decrease of 9.4% from the same quarter of 2020. Of these suicide deaths, 79.3.% (107) were male. See tables 2a, 2b, 13, 14, 15 and 16. 

All unnatural deaths, which includes deaths from intentional self-harm, must be referred to the Coroner's Office. However, the Coroner's Court did not hold public hearings at various stages over the course of the COVID-19 pandemic, as a consequence of the restrictions imposed. This would impact on the number of deaths registered and subsequently assigned an Underlying Cause of Death of intentional self-harm. Users should bear this in mind when comparing percentage increases/decreases from 2020-2021 quarterly results. 

Deaths assigned an Underlying Cause of Death of External causes of injury and poisoning (including deaths from road traffic accidents and deaths from intentional self-harm), are likely to be underrepresented as such deaths are reported to the Coroner’s Office for further investigation. This can then result in such deaths being registered late (more than three months after the date of occurrence) and therefore have not yet been reported to the CSO. In addition, public hearings in the Coroner's Court have also been limited as a consequence of the restrictions imposed due to the COVID-19 pandemic and this may also have resulted in deaths being registered late.

Dementia and Alzheimer's:

In Quarter 4 2021, there were 498 deaths due to dementia of which 307 (or 61.6%) were female. There were 157 deaths due to Alzheimer's of which 116 (or 73.9%) were female. See tables 18 and 19.

COVID-19

There were 485 deaths due to COVID-19 recorded in quarter 4 2021, 278 of which were male and 207 which were female. Of these, 301 (62.1%) were aged 75 and over.  

There were 484 of these COVID-19 deaths where the virus was identified while there was 1 death where the virus was not identified. See tables 20, 21 and WHO ICD-10 classification of COVID-19 and WHO emergency use ICD codes for COVID-19 disease outbreak 

It is important to note that there will be a number of deaths where COVID-19 will not be assigned as the Underlying Cause of Death (UCOD) and therefore, the COVID-19 deaths in this analysis, will vary from those put into the public domain by the Department of Health. This is because the figures published in the Vital Statistics quarterly releases are based on the narrative of cause of death that is reported, in electronic format, on the death certificate once the death is registered with the General Registration Office (GRO) and subsequently forwarded to the CSO for processing. All deaths that are registered are assigned an Underlying Cause of Death in line with the World Health Organization (WHO) International Statistical Classification of Diseases and Related Health Problems (ICD-10). This UCOD is assigned using the WHO selection rules and is dependent on the hierarchy of diseases or conditions and their placement on the medical death certificate. There are also instances where COVID-19 is mentioned in the narrative of the death certificate but where the UCOD is not COVID-19.

The mean average age of death was 77.7 years while the median age was 79.3 years for all deaths due to COVID-19 in quarter 4 2021.

COVID-19 Vaccine

In Quarter 4 2021, a death due to COVID-19 vaccines causing adverse effects in theraputic use was registered for the first time. This death occurred in the 75-84 age group: See tables 14, 15 and 16 in addition to: WHO ICD-10 classification of COVID-19 and WHO emergency use ICD codes for COVID-19 disease outbreak

Age at death

The leading cause of death in quarter 4 2021 varied widely by age-group. For those aged from 15 to 34 years, external causes of death including accidents, suicide and other ranked first (91 deaths). Among individuals aged 35-74, malignant neoplasms (cancer) was the leading cause (1,182), while for those aged 75 and older, it was diseases of the circulatory system (1,698 deaths). See table 14.

There were 6,992 deaths of persons aged 65 and over registered in quarter 4 2021. This represents an annual death rate of 37.7 per 1,000 population aged 65 and over and represents an increase of 4.5 when compared with the corresponding quarter of 2020. See tables 3, 4 and figure 5.

The average (mean) time-lag between the date of death and the registration of death was 78 days while the median (mid-point in data) was 35 days for quarter 4 2021.

Natural Increase

The natural increase in the population (births minus deaths) for quarter 4 2021 was 7,624. This represents a rate of 6.1 per 1,000 population, 1.1 more than quarter 4 2020. See tables 3, 4 and figure 1.

Marriages

The number of marriages registered during the quarter was 5,385 of which 140 were same-sex marriages. This is equivalent to an annual marriage rate of 4.3 per 1,000 population, an increase of 1.7 from quarter 4 2020. See tables 3 and 4.

Same-sex marriage legislation came into effect on the 16th November 2015.

Population

The estimated population (usual residence) in April 2021 was 5,011,460.

Background Notes

Introduction

This report has been prepared by the Central Statistics Office for the Minister for Social Protection in accordance with the provisions of Section 2 of the Vital Statistics and Births, Deaths and Marriages Registration Act, 1952 and Government Order SI 831 of 2007.

Methodology:

All figures in this report are classified by quarter of registration and are provisional. 

Deaths are classified to the World Health Organisation (W.H.O.) International Classification of Diseases, 10th Revision (ICD10). 

The Underlying Cause of Death (UCOD) has been defined by the World Health Organisation (WHO) as

(a) the disease or injury which initiated the train of morbid events leading directly to death, or (b) the circumstances of the accident or violence which produced the fatal injury’, and is selected for routine single-cause tabulation of mortality statistics. 

All deaths registered in the quarter have been included in the statistics and some cases have been assigned a provisional cause of death pending the outcome of further enquiries. 

Caution should be exercised if directly comparing these mortality statistics with those of previous quarters as this data is provisional and subject to change. Final data is published in our annual reports and is based on the date the death occurred while the quarterly releases are based on the date the death was registered.

Inquest deaths are coded manually due to the nature of the reporting of these cases.  The difficulty lies in automatically assigning a verdict from a broad range of verdicts open to a Coroner or jury which include accidental death, misadventure, suicide, open verdict, natural causes (if so found at inquest) and in certain circumstances, unlawful killing. A provisional cause of death may been assigned to a number of inquest cases pending the outcome of further enquiries. 

From the 1st January 2018 the CSO is using automated software called IRIS for selecting the underlying cause of death code. The coding system has been developed and is maintained by the IRIS core group to code mortality data and is the preferred coding tool for European countries. All W.H.O. updates to the ICD-10 classification are included in the software according to the WHO timelines.

Stated Parity 

Refers to the number of previous live births excluding any not stated.

Births Classified by Area of Residence of Mother

From January 2018 onwards where births take place within the State and the area of residence of the mother is outside the State, the area of residence of the mother is recorded as outside the State. Prior to this, births where the mother was residing outside the State were assigned to the location where the birth took place within the State.

NUTS Regional Authority Areas 

From the 1st January 2018 an Amendment of the Eurostat Nomenclature of territorial units for statistics (NUTS classification) legislation came into effect. This reflects the new regional assembly structure and the other changes to the Local Government Act for Tipperary North, Tipperary South and Limerick and Waterford City and County Councils. The composition of the new Regions breakdown is outlined below.

Up to December 2017 regional classifications were based on the NUTS classification used by Eurostat. The NUTS3 regions corresponded to eight Regional Authorities established under the Local Government Act, 1991 (Regional Authorities) (Establishment) Order, 1993, which came into operation on 1 January 1994. The NUTS2 regions, which were proposed by Government and agreed by Eurostat in 1999, are groupings of the NUTS3 regions. The composition of the regions is set out below.

The European Union (EU) Countries

 The United Kingdom withdrew from the EU on the 31 January 2020. From the 1 February 2020 the EU is reduced to 27 Member States.

 As the United Kingdom was part of the EU for one month in 2020 - it is included as part of the EU 28 for quarter 1 2020.

Civil Partnerships

Civil Partnership legislation was introduced in Ireland on the 1st January 2011. From 16th November 2015, when same-sex marriages legislation came into effect, same-sex couples no longer had the option to apply for a Civil Partnership.  However, couples that had given the required 3 months' notice of a Civil Partnership prior to 16th November 2015 were still entitled to have a Civil Partnership ceremony after that date.  

Same-Sex Marriage Legislation

The Marriage Act 2015 (No. 35/2015) effective from the 16th November 2015 (S.I. 504/2015) refers, allows parties of the same sex to marry.

Usual Residence Population Concept

Rates for quarter 4 2021, were calculated using the relevant population data in April 2021.

For the purpose of this release the population concept of usual residence has been used, i.e. all persons usually resident and present in the State on census night, plus absent persons who are usually resident in Ireland, but are temporarily away from home and outside the State on Census night.  All persons are classified according to the region of their usual residence.

Revision of Rates

Rates for Births, Deaths and Marriages were revised in quarter 4 2017 for intercensal years 2012 to 2015, based on revised estimated usual residence population data. Rates for 2016 were also revised based on updated Census of Population usual residence population figures.  

Technical Notes:

Live Births

The terms used in relation to live births are defined as follows:

A live birth: is defined as the complete expulsion or extraction from its mother of a product of conception, irrespective of the duration of pregnancy, which, after such separation, breathes or shows any other evidence of life, such as beating of the heart, pulsation of the umbilical cord, or definite movement of voluntary muscles, whether or not the umbilical cord has been cut or the placenta is attached.

Parity: Parity is the number of previous live born children to a woman.

Birth Order: Birth order is the number  of live born children a woman has in the order in which they are born.

Parity and Birth order: A first birth relates to parity 0 or birth order 1 i.e. birth order = parity + 1.

Crude birth rate (CBR): The number of live births divided by the total population at that age (or age-group), multiplied by 1,000.

Age specific fertility rate (ASFR): The number of live births at a certain age (or age-group) divided by the female population at that age (or age-group) multiplied by 1,000.

Total period fertility rate (TPFR): The sum of the ASFRs, divided by 1,000 i.e. the expected number of children a woman will have in her lifetime based on the fertility of that year.

Formula for technical notes

Age specific reproduction rate (ASRR): The number of female live births at a certain age (or age-group) divided by the female population at that age (or age-group), multiplied by 1,000.

Gross reproduction rate (GRR): The sum of the ASRRs divided by 1,000 i.e. the expected number of daughters a woman will have in her lifetime based on the fertility of that year.

Net reproduction rate (NRR): The sum of the products of the ASRR's and the expected number of female years to be lived at that age, divided by 1,000 i.e. the GRR adjusted for female mortality.

The difference between the gross and the net reproduction rates is an indicator of female mortality.

Average age at maternity: The sum of the products of the ages at maternity of live births and their ages divided by the number of live births.

 Formula 2

Average age at maternity of first birth: The sum of the products of the ages at maternity of first live births and their ages divided by the number of first live births.

Childbearing years are regarded as between 15 and 49.

Live birth to mothers less than 15 are included in the age 15 category and are divided by the age 15 population. Similarly, live births to mothers aged greater than 49 are included in the age 49 category and are divided by the age 49 population.

Average of mean ages: Live births where the age at maternity is not stated are excluded from the calculation.

Deaths

Crude death rate (CDR): The number of deaths divided by the total population at that age (or age-group), multiplied by 1,000.

Standardised death rate : the (age) standardised death rate is a weighted average of age-specific mortality rates. The weighting factor is the age distribution of a standard reference population. The standard reference population used is the European standard population as defined by Eurostat in 2012. The new European Standard Population (ESP) is the unweighted average of the individual populations of EU-28 plus EFTA countries in each 5-years age band (with the exception of under 5 and the highest age group of 95+). The ESP is calculated on the basis of the 2010-based population projections, averaged over the period 2011-30.  

Stillbirths, Infant Mortality and Maternal Mortality

Stillbirth:  Stillbirth weighing 500 grams or more or at gestational age of 24 weeks or more, this definition applies to stillbirth figures from 1995 onwards.

Late foetal death: Foetal death at or over 28 weeks gestation, this definition was applied up to 1994 inclusive.

Early neonatal death: Death at ages under 1 week live born infant.

Perinatal deaths: Stillbirths or late foetal deaths plus early neonatal deaths.

Late neonatal deaths: Death between the ages of 1 week and 4 weeks of live born infant.

Neonatal death: Death at ages under 4 weeks of live born infant.

Post neonatal death: Death between the ages of 4 weeks and 1 year of live born infant.

Infant death: Death at ages under 1 year of live born infant.

Maternal death: A maternal death is the death of a woman while pregnant or within 42 days of termination of pregnancy, irrespective of the duration and the site of the pregnancy, from any cause related to or aggravated by the pregnancy or its management, but not from accidental or incidental causes.

Stillbirth rate: Stillbirths divided by total live births plus stillbirths, multiplied by 1,000.

Late foetal mortality rate: Late foetal deaths divided by total live births plus late foetal deaths, multiplied by 1,000.

Early neonatal mortality rate: Early neonatal deaths divided by total live births, multiplied by 1,000.

Perinatal mortality rate: Perinatal deaths divided by total live births plus stillbirths, multiplied by 1,000.

Late neonatal mortality rate: Late neonatal deaths divided by total live births, multiplied by 1,000.

Neonatal mortality rate: Neonatal deaths divided by total live births, multiplied by 1,000.

Post neonatal mortality rate: Post neonatal deaths divided by total live births, multiplied by 1,000.

Infant mortality rate: Infant deaths divided by total live births, multiplied by 1,000.

Maternal death rate: Maternal deaths divided by total live and stillbirths multiplied by 100,000.
